General Information and Eligibility
If you are employed on a full-time basis [regularly scheduled to work 30 hours or more per week] the following programs are offered:
Employee Term Life
Employee Accidental Death & Dismemberment
Dependent Life
Long Term Disability
These programs are fully insured and are underwritten by MetLife Insurance Company. You participate in the following programs:
Employee Term Life and Employee Accidental Death & Dismemberment:
Salaried Employee (Excluding Employees of Erskine College) - $50,000
Hourly Employee (Excluding Employees of Erskine College) - $20,000
Erskine College Employees - $10,000. Contact Human Resource Office for information on additional coverage.
Long Term Disability (Excludes Employees of Erskine College. Contact Human Resource Office for details of Erskine's Disability programs.)
Dependent Life Insurance. If you are married your Spouse will have $5,000 Term Life Insurance. If you have unmarried dependent children less than age 19 (25 if a full-time student) coverage will be:
Child age 15 days to 6 months - $200
Child 6 months and older - $2,500.
Medical Insurance
The Medical program is primarily self-insured and uses Blue Cross Blue Shield as its Preferred Provider Network. BCBS of South Carolina acts as the Third Party Administrator (TPA) for eligibility, claim processing, case management and the Prescription Drug Program.
BCBS will provide participant services through "My Insurance Manager" (http://www.southcarolinablues.com/). Participating physicians and facilities can be located through "The BlueCard Program" (http://www.bcbs.com/).
The Plan provides Case Management in several voluntary programs for which the Plan pays for your participation:
Weight Management
Quit for Life - A special program for members who want to stop using tobacco
Maternity
HealthStat - Located at Erskine College, a nurse practitioner is available for several hours per week. The services are available for all Plan Participants.
As a Church Plan, the General Synod Plan is not subject to all requirements that govern non-church plans. It is the intent of the Board of Benefits to generally follow the intent of these laws. Accordingly, there are several special provisions of the medical plan that you need to review:

The Dental program is self insured and uses Kanawha HealthCare Solutions as the Third Party Administrator for eligibility and claim processing. Participants are able to access information on claims through the Kanawha Web-Site (http://www.khsonline.com/). Be sure to review:
